When the vendor requires terms and conditions that differ from our basic (SPA) contract then we must use a contract addendum (SCA) to ensure that the vendor's terms and conditions align with Texas state and UH System requirements.
Insurance Terms Approval Form is used to acquire approval of or revisions to non-standard agreement insurance terms. Send this form to UHCLProcurement@uhcl.edu for processing.
