Workplace Accommodations

In accordance with SAM 02.E.09, Reasonable Workplace Accommodations for Employees with Disabilities, UHCL affords equal employment opportunity and provides reasonable workplace accommodations to qualified employees with disabilities as well as persons with sincerely held religious beliefs and women affected by pregnancy. When an employee makes a request for a reasonable accommodation that involves performance of their job, HR works with the employee and the employee's supervisor to ensure that an appropriate accommodation is provided that meets the individual's disability-related needs and enables the individual to perform the essential functions of their position.

In addition to consulting with their medical provider, an employee may find the Job Accommodation Network (JAN) a helpful resource for learning more about the Americans with Disability Act (ADA) and exploring accommodation options. The U.S. Department of Labor (DOL) is another excellent resource for seeking more information about workplace accommodations. 

To request an accommodation, complete the Request for Workplace Accommodation Form. Medical accommodations also require the Medical Inquiry Form (completed by a physician) and the Medical Release Authorization Form.

Submit completed forms to Human Resources. Please note employees may provide these forms to HR directly and let their supervisor know they have started the process to make a reasonable accommodation request with HR (without giving their supervisor the full form and related medical information). Supervisors that receive an accommodation request must immediately notify HR.
