COVID-19 Diagnosis/Symptoms Protocols

Employee and Supervisor

Employees, students, and community members should not come to campus if they are exhibiting any signs or symptoms of COVID-19, or if they have been in close contact (within 6 feet for 15 minutes or more) with another person that has tested positive for COVID-19.

If an employee or student should become ill, or come into close contact (within 6 feet for 15 minutes or more) with a positive COVID-19 person while on campus, they will be required to go home immediately.

Employee Steps

  • If at work, go home immediately and avoid coming into close contact with anyone.
  • Contact your supervisor via phone as soon as possible to notify them.
  • Complete the employee form: Report Exposure, Symptoms, or Diagnosis within 24 hours for going out of work, including for quarantine, sign(s) or symptom(s), and/or suspected diagnosis.
  • Contact your health care provider for diagnosis, treatment and/or testing, as necessary.
  • Stay home (except to seek medical care) and follow theSelf-Quarantineprotocols.

  • Employee reports for going out of work due to Exposure Quarantine, Signs/Symptoms and Positive Cases (Isolation) are all contained in one form titled Report Exposure, Symptoms, or Diagnosis. Those reports as well as the Request to Return to Work after Quarantine or Isolation form submissions are reviewed and followed up by EHS and supporting Contact Tracers. Employees requesting guidance from a counselor may contact and continue follow-up with Counseling Services.
  • Do not send an email notification with any personal or medical information. The online forms are the secure channel for submitting personal or medical information.
  • Provide a good daytime contact number and check your phone and work email for communication from Contact Tracers.
  • If you are suspected/presumed to have COVID-19 and later receive a positive test result, notify your supervisor as well as contact tracer of this development.
  • Cooperate with contact tracers to provide information regarding locations you have been on campus, and others with whom you have had close contact. This will include the two days before you started noticing symptoms.
  • Complete the Request to Return to Work after Quarantine or Isolation form for further instruction or approval.
  • Do not return to campus until written approval is received.

Activity Log

Keep a log of your locations on campus. If you should develop symptoms of COVID-19, you will be asked for a list of locations visited on campus, starting two days before noticing symptoms. A notepad on your desk or work email calendar may be used, and should be accessible to your supervisor. This will assist if you should become ill and are unable to access it or reply.

Supervisor Steps

  • If the employee is at work when you are notified, make sure the employee is separated from others and immediately sent home.
  • If the employee is at home, advise them to stay at home and follow the protocols above. Also, find out when the employee was last at work.
  • Have your employees fill out the Report Exposure, Symptoms, or Diagnosis form online within 24 hours of leaving work for Contact Tracers to follow up with them. Do not email reports or any personal, medical information. If an email is needed, only send a name and good contact number for the employee.
  • There is only one report form for leaving work, whether it is Quarantine after potential exposure/close contact with someone positive, Sign(s) and/or Symptom(s), or Confirmed Positive cases.
  • Contact Tracers will determine if employees really do need to be quarantined, and will submit a Work order to FMC for any areas needing disinfected.
  • Have employees fill out the Return to Work after Quarantine or Isolation form after 14-day quarantine or isolation period. Tracers will review and give approval to return.
  • Contact Human Resources if you have a question regarding your employee's leave options.
  • Consider common areas, common tools and equipment touch points. Identify alternative locations employees may work to avoid being in close proximity to each other. Disinfect shared items and touch points after each use or shift as appropriate.

If the employee has tested positive or been diagnosed with COVID-19, the supervisor should also:

  • Advise the employee to follow the protocols for employees; also, find out when the employee was last at work.
  • Close off areas used by the infected employee, including common areas. CDC guidance advises that cleaning/disinfecting should not begin for 24 hours from employee's presence; If the infected individual has not been present for 7 days or more, additional cleaning and disinfection is not necessary besides increased routine cleaning and disinfecting of all high-touch surfaces already in place.
  • If that employee worked in close proximity to other employees (within 6 feet for 15 minutes or more) for that day and the two days prior to the employee noticing symptoms, notify employee's co-workers of a potential exposure, but do not identify the employee by name
  • Advise co-workers that they will be contacted if there is a need to quarantine as a precautionary measure and that they do not need to take any specific action unless notified, but they should continue to monitor their health for the presence of any Coronavirus Symptoms and follow the Prevention and Workplace Prevention practices.
  • Send home any potentially exposed employees per contact tracer instructions, and instruct them to complete the Report Exposure, Symptoms, or Diagnosis form and to follow the Self-Quarantine protocols.
  • Inform all employees who have tested positive that they will need to complete the Request to Return to Work after Quarantine or Isolation form and receive written approval before they can return to campus.
