Adult Informed Consent Form - This informed consent form is used only for individuals 18 and over.

Adult Informed Consent-Min Risk-Injury Disclaimer - This form is used if physical risk is a possibility.

  Online Survey Consent
 

Online Survey CPHS Application and Informed Consent Instructions - This document provides instructions regarding online surveys and what types of information to include in the CPHS application and informed-informational research document.

Informed-Informational Research Document - This form is used for projects that have requested and received CPHS waiver for documentation of informed consent. Participants' signatures are not required.

Online Informed Consent - Use this informed consent form for online survey projects.

  Research Involving Children
 

Because of differences in children's age, language comprehension, maturity, etc., multiple assent-consent form templates are available. We have included a series of links to forms for children of different ages with brief descriptions indicating when each should be used.

  • Parental Consent form - Use for non-education research with children aged 7-17 when separate parental consent and child assent forms are needed.

Assent forms used for non-education research with children under 12 or children aged 13-17 and separate parental consent and child assent forms are needed.

  Educational Research
 

While not specifically required by the federal regulations, these are forms designed for research on the effectiveness of educational techniques.

Parental Consent Form for Child under 7 years old for Education Research - Use this parental consent form for children under the age of 7 who are involved with education projects. Only parent's signed consent is required; however, verbal assent from the child should also be obtained.

Consent/Assent Form for Parent and Child aged 7-12 for Education Research - Use this combined child assent and parental consent form for children aged 7-12 involved with education projects.

Consent/Assent Form for Parent and Child aged 13-17 for Education Research - Use this combined child assent and parental consent form for children aged 13-17 involved with education projects.
