Tutor Request Form

Tutoring is usually scheduled in 45 minute sessions in the study carrels behind SSCB 3101.02.

  • Mon – Thu: 3:00 – 8:00 pm
  • Fri: 12:00 – 5:00 pm

You can request multiple tutoring appointments at one time for a particular subject.

  • Example 1: April 14 - 18 any afternoon until 6:00 pm
  • Example 2: ASAP 3:00 - 4:30 pm
  • Example 3: First Two Monday appointments after 4:00 pm.

You can expect to receive an email with information about your tutoring appointment from the Dean of Students within 1 business day. Should you need to cancel this appointment, please email or call the Dean of Students immediately.

When you submit your form, your data is sent securely.
If your form has been submitted successfully, you will see a Thank you message. 

State law requires that you be informed of the following: (1) with few exceptions, you are entitled on request to be informed about the information the University collects about you by use of this form; (2) under sections 552.021 and 552.023 of the Government Code, you are entitled to receive and review the information; and (3) under section 559.004 of the Government Code, you are entitled to have the University correct information about you that is incorrect.
